One total testosterone result is not a diagnosis. The panel answers three questions: is the level genuinely low, why is it low, and is treatment safe for you.

Testosterone moves through the day, so when it gets measured matters.

  • Total testosterone — drawn in the morning, when it reads highest, and repeated if the first result sits on the line
  • Free testosterone and SHBG — how much of that total is available to your tissues
  • LH and FSH — whether the shortfall starts in the testes or in the signal from the pituitary
  • Estradiol — some testosterone converts to estrogen, and that balance matters on treatment
  • Prolactin and further pituitary studies where the picture calls for them

Injection, cream or pellet — the choice shapes your week, not just your dose.

All three raise testosterone. They differ in how steady the level stays, how often you think about it, and how fast a dose can change. We prescribe FDA-approved testosterone preparations through licensed pharmacies, physician-supervised throughout.

The most common route, and the easiest to fine-tune between draws.

What counts as low testosterone?

A number and a picture, not one or the other. We look at total and free testosterone from a morning draw, confirm a borderline result on a second sample, and read it against your symptoms and the rest of the panel. Two men with the same number don’t always get the same answer.

Which is better — injections, cream or pellets?

None of the three is better in the abstract. Injections give the most control between labs. Creams keep levels flatter day to day but ask for daily attention and care around close contact. Pellets remove the daily step, at the cost of flexibility once they are placed. You pick with your provider once the labs are back.

How often will I need bloodwork once I start?

Testosterone is a monitored treatment, so labs repeat after you begin and then on a set schedule. We follow your testosterone level, hematocrit, estradiol and PSA, and the dose moves with those results.

Can I stop testosterone therapy later?

Yes. Your own production is suppressed while you are on treatment and takes time to recover afterwards, and if the deficiency was real the original symptoms usually return. It is a decision worth taking deliberately at the start.

Do you take insurance, and what does TRT cost here?
