Everything published here is general information about services the practice offers. It is not medical advice, it does not create a doctor–patient relationship, and it is not a substitute for an evaluation. Enquiries and appointments
Sending a form does not book an appointment and does not create any obligation on either side. Someone from the office will call you back to arrange things. If your situation is urgent, call rather than using a form. If it is an emergency, call 911.
Payment
LifeBoost is a self-pay practice and does not bill insurance. You will be quoted a price in writing before treatment begins, and no price is quoted on this website because it depends on what is actually indicated for you.
Content and trade marks
The text, photographs, design and code of this site belong to LIFEBOOST LLC or are used with permission. You may read, print and share pages for your own personal use. You may not republish, sell or systematically copy the content, or present it as your own.
