What it actually is

Semaglutide, in plain terms.

It is a GLP-1 receptor agonist. GLP-1 is a hormone your gut releases after you eat; part of its job is telling the brain the meal has landed. Your own version breaks down in minutes. This one keeps the signal running for days.

Mechanism

Three things it changes

One signal, doing its normal job for longer than it naturally would.

  • Appetite between meals. The pull toward the next snack quiets down. Patients mention this before the scale.
  • Fullness during a meal. You reach enough sooner and stay there longer.
  • Gastric emptying. The stomach clears more slowly — which extends the fullness, and is why most side effects are digestive.

Honest limits

What it doesn't do

It is not a fat-burner and not a plan on its own. What it does is make an eating pattern sustainable that willpower alone was not — a different claim from doing the work for you.

  • Protein and resistance training decide how much of what you lose is fat rather than muscle.
  • It does not fix an underactive thyroid, insulin resistance or a hormone deficiency. Those are found on labs and treated on their own terms.

Nothing is prescribed before your labs and a results review — someone here goes through the numbers with you, by phone or at the office, rather than emailing a PDF, Dr. Stratt consults on what gets prescribed, and you have the price in writing before you decide. After that, most bad experiences with this medication trace back to going up too fast. How fast the dose goes up is the part that decides how you feel. That's the part we manage in person.

Your first dose

You start below the dose expected to do the work — the gut needs time to adapt. We show you the injection in the office: same day each week, under the skin.

Stepping up

The dose moves up in stages, with weeks between increases. How fast depends on how you are tolerating it, not on a calendar applied to everyone. Holding longer at a dose is a normal decision.

Check-ins at the office

You come in to 7035 Beracasa Way. Weight, blood pressure, what you are eating, what side effects you have had, what the labs say. An online form can't tell us how you're actually tolerating the dose. That's what the visit is for.

Maintenance, and the exit

At some point the dose stops climbing. Where that lands, how long you stay there and what happens afterward get decided with you — at the start, not improvised later.

This is a prescription medication with real side effects and real reasons we would not prescribe it. We go through both with you before you start.

  • A history of pancreatitis or significant gallbladder disease
  • Certain gastrointestinal conditions, reviewed case by case
  • Labs pointing to a thyroid or hormone problem we should treat first

  • Hydration, fibre and protein guidance that heads most of it off
  • Repeat labs where indicated

How is semaglutide taken?

A once-weekly injection under the skin — abdomen, thigh or upper arm — given at home on the same day each week, with or without food. We show you how at the office before your first dose, and you can call us if one does not go as expected.

How soon will I notice anything?

Most people notice the appetite change before the scale moves, often within the first few weeks. The starting dose is deliberately low, so the early effect is usually mild, and weight change is rarely a straight line. We look at the trend with you in person.

What side effects should I expect?

Most are digestive: nausea, reflux, constipation or loose stools, usually worst in the days after a dose increase. Going up slowly is what keeps them manageable, and holding at a dose is a normal adjustment rather than a failure. Severe or persistent abdominal pain is different — call us the same day.

What happens if I stop taking it?

Appetite regulation drifts back toward where it was, and weight can follow. That is not a reason to avoid the medication — it is a reason to decide early what comes after it: nutrition, activity, and any hormonal issue your labs turned up. We discuss the exit at the start.

Do you take insurance, and when do I find out the cost?
