What this policy covers
This policy covers information collected through lifeboostmd.com, operated by LIFEBOOST LLC, 7035 Beracasa Way, Suite 102, Boca Raton, FL 33433. It does not cover the medical records held about you as a patient of the practice, which are governed by the practice's own records policies and by applicable Florida and federal law. If you want to know how your clinical records are handled, call the office on (561) 922-9967 and ask.
What we collect
If you fill in a form on this site, we receive the information you type into it: your name, telephone number, email address, which service you are asking about, and anything you choose to write in a free-text field. We also record which page the form was sent from.
Please do not use these forms to send detailed medical information. They are for arranging a conversation, not for taking a history. Anything clinical is better discussed by phone or in the office.
The site also uses Google Analytics, which sets cookies and collects standard usage data — pages viewed, approximate location, device and browser type, and how you arrived. We use it to see which pages lead people to call or enquire.
What we do with it
Form submissions are emailed to the practice so that someone can call you back, normally within one business day. We use your details to respond to your enquiry and to arrange care if you decide to become a patient.
We do not sell your information. We do not share it with third parties for their own marketing.
Who else is involved
Two service providers necessarily process data on our behalf: our web host, which stores and transmits the site and the messages sent through it, and Google Analytics, which processes site usage data. Google's handling of that data is governed by its own privacy policy.
Text messages
If you tick the consent box on a form, you are agreeing that we may contact you by phone or text about your enquiry. Message frequency varies. Reply STOP to opt out at any time. Consent to texting is never a condition of becoming a patient.
How long we keep it
Enquiries that do not become appointments are kept only as long as they are useful for following up, and are then deleted. Records relating to care you actually receive are retained for the period Florida law requires for medical records.
Your choices
You can ask us what enquiry information we hold about you, ask for it to be corrected, or ask for it to be deleted. Call the office on (561) 922-9967. You can block or delete cookies in your browser settings, and you can opt out of Google Analytics using Google's own browser add-on. Neither will stop you using the site.
Children
This site is not intended for children, and we do not knowingly collect information from anyone under 18 through it.
Changes
If this policy changes, the updated version will be posted here with a new date at the top.
